Pet Sitting Agreements

PLEASE READ CAREFULLY AND CLICK THE CONSENT BOX ABOVE TO AGREE TO TERMS

I authorize Sitter to have access to my home in order to care for my pet(s).

If my pet(s) becomes ill, I understand that Sitter will notify me immediately, but if in the case that I cannot be reached in a critical emergency, I authorize Sitter to transport my pet(s) to a veterinarian or Animal Emergency Hospital to receive medical treatment and authorize medical treatment. I agree to pay all expenses incurred for treatment and service. In addition to any third-party expenses incurred at the hospital, vet facility Services Agreement or otherwise, Sitter will be compensated for emergency services at an hourly rate of __$55__/hr.

Billing for such services shall begin when the pet is determined to need emergency services and shall be billed in half hour increments.
The minimum increment for emergency services requiring transportation is 1.5 hours.

I have read and agree with the terms and conditions of the Emergency Authorization Notice attached hereto and made a part of herein regarding any limitations on emergency services, any non-resuscitation, or the necessity of euthanasia.

I authorize Sitter to transport my pet or pets as necessary to carry out the purposes of this Services Agreement or to protect the health and safety of my pet. I acknowledge that transportation fees for scheduled services must be paid in advance.

I certify that my pet has all immunizations and permits mandated or required by the governmental agencies where my pets are primarily located.

I acknowledge that I must provide a leash and collar for any dog which will be taken out of my home, such leash and collar to be reasonably acceptable to a representative of Sitter.

I acknowledge that I must create and client account through Lord Of The Leash Pet Solution's third party application called Time To Pet and any communication with my Pet Pro, visit reports, questions, comments and concerns will be recorded through Time To Pet.

I acknowledge I am not permitted to request my Pet Pro's private information (phone number, emails or addresses) outside of a serious emergency and all communications will be routed through Time To Pet or the managing staff.

Furthermore, I acknowledge that it is my responsibility to provide suitable transportation carriers for any of my pets when I have selected transportation services.

I hereby hold Sitter, its owners, officers, directors, employees subcontractors and agents (collectively "Sitter Releasees") harmless from and against any and all actions, causes of action, warranties, claims, suits, debts, sums of money, covenants, guarantees, promises, damages, judgments and claims of any kind or nature in connection with or arising out of this Services Agreement and the transactions and services contemplated thereby.

I further agree to indemnify Sitter Releasees from and against any and all third party actions, causes of action, warranties, claims, suits, debts, sums of money, covenants, guarantees, promises, damages, judgments and claims of any kind or nature in connection with or arising out of the performance of this Services Agreement, including, without implied limitation, costs and fees associated with emergency medical care for my pets, unpaid grooming or vet care charges ordered by me, or any claims that my pet injured or caused damage to any third party while in the care of Sitter.

This Services Agreement will remain valid for future service, with the exception of any agreed changes in fees, frequency, or total number of visits.

This Services Agreement may be terminated by either party on notice to the other party and the payment by the Client of any outstanding charges. Notwithstanding any such termination, the provisions of this Services Agreement relating to release of liability and indemnification shall survive indefinitely.

Cancellation/Early Return Policy
Because we are dedicated to you when you make a reservation and will turn away other reservations, therefore we require a non-refundable deposit of half of your total booking request to confirm your booking. Without the deposit your booking is NOT confirmed.
The deposit is a retainer and cancellation fee rolled into one.

Any and all deposits made for reservations for services whether they fall on a Major Holiday, during a natural disaster/storm or not (see lordoftheleash.com for a list of Major Holidays) will not be refunded or credited.

Additional Charges/Services: $10.00 additional for Last Minute Visits (24 Hours Before Service/Day Of)

Additional Charges/Services: $25.00 fee for any last minute canceled/no call/no show in-home training session, dog walking or day visit

Delay of Service Fee Attempts of gaining entrance into the property through a security guard/ guard. If the entrance is denied and advise that we are not on the list.

This fee will apply, and services may not be able to be performed or shortened. If this occurs more than once during time of service, the following charges will occur.
• 1st Deterrent – No Charge
• 2nd Deterrent – $5.00
• 3rd Deterrent – $10.00
• 4th Deterrent – $15.00 or more
• Additional Visit Charge if we must return.

Hurricane Season Agreement

As pet owners, we need to make sure we are prepared for hurricane season as effectively as possible.

In the event of a Hurricane or Tropical Storm where curfews, shelter in place, or evacuations are ordered during the time your pet(s) are in our care, we will be unable to provide services.

We want the best situation possible for your pets during this time, so we are requesting additional information.

The following people in Emergency Fields and/or locations listed in the section below have been notified before your departure and will agree to take your pet(s) during the duration of the storm:

All people/locations listed above in the Emergency Contact Field will be contacted and you will be notified of the storm procedure.

Please note the following must also be provided:
“Evac Pac” for Your Pet - should we need to leave your home quickly in case of an emergency, we will need to have a grab bin or bag with your pets supplies.

You should include:
• Extra Leash/Harness/Collar with Identification • Towels/Blankets
• At least a week’s worth of pet food/fresh water
• An extra set or disposable food/water bowls • Disposable litter pans (for cats)
• A t-shirt or blanket that smells like you
• Poop Bags • Identification Folder/Binder with Current Photos
• Copy of Medical Records
• Soft or Hard Crate
• Addition Pet Medication (if not refrigerated)

I have read and understand all of the above information. All of the people/locations I have listed above either in the Emergency Contact Fields have been notified before my departure and will take custody of my pet(s) during the storm. I understand that failure to provide the supplies listed above before my departure will result in a $200 errand fee in addition to the cost of the supplies to be paid to the sitter immediately.
